Qualifications

• Active, unrestricted Registered Nurse (RN) licensure in Connecticut.
• 1–2 years nursing experience in SNF/LTC or sub-acute rehab preferred.
• Admissions, discharge coordination or desk nursing experience is desirable.
• Hands-on EMR proficiency for rapid chart initiation.
• Excellent communication and interpersonal skills.

Responsibilities

• Lead evening admissions and onboarding for new residents (3–11 PM).
• Perform initial nursing assessments and admission documentation.
• Reconcile hospital discharge summaries and orders for continuity.
• Coordinate with discharge planners, care teams, and families.
• Develop preliminary care plans and relay needs to on‑duty supervisors.
• Orient residents and families to facility policies and routines.
