Accounting Search is seeking an Admissions Nurse (RN) in Connecticut for an evening shift (3:00 PM–11:00 PM) to oversee clinical intake and admissions processes. The role coordinates with hospital discharge planners, physicians, leadership, and families to ensure smooth transitions and accurate chart setup.

Qualifications

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC/sub-acute rehab preferred.
• Experience handling admissions or discharge coordination is highly desirable.
• Experience with EMR systems (e.g., PointClickCare) and accurate chart initiation.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM).
• Perform initial nursing assessments, skin evaluations, and admission documentation.
• Reconcile discharge summaries, orders, and pharmacy profiles for medication continuity.
• Collaborate with discharge planners, care teams, and families to arrange room setups and equipment.
• Develop preliminary individualized care plans and brief supervisors on resident needs.
• Orient residents and families to policies and routines to ease transitions.
