Job summary

Talent With Heart in Connecticut is seeking an Admissions Nurse (RN) to oversee and streamline clinical intake for incoming residents, coordinating with hospital discharge planners, physicians, leadership, and families to ensure a safe transition into the center.

Focused on evening admissions (3:00 PM–11:00 PM), the role covers initial assessments, medication reconciliation, and rapid chart setup so floor staff can care for residents from day one.

Qualifications

• Active, unrestricted RN licensure in Connecticut.
• 1–2 years nursing experience in SNF/LTC/sub-acute rehab preferred.
• Experience handling admissions, discharge coordination, or desk nursing is desirable.
• EMR proficiency with a system like PointClickCare for rapid chart initiation.
• Strong organizational, prioritization, and interpersonal skills.

Responsibilities

• Lead clinical intake and onboarding procedures for evening shift (3:00 PM – 11:00 PM).
• Conduct initial nursing assessments, skin evaluations, and admission documentation upon arrival.
• Reconcile hospital discharge summaries, orders, and pharmacy profiles for immediate medication continuity.
• Collaborate with discharge planners, care teams, and families to arrange room setups and equipment.
• Initiate preliminary care plans and communicate resident needs to on-duty supervisors.
• Orient new residents and families to policies, care team structures, and routines.
