Job summary

Workforce Recruiter is seeking an Admissions Nurse (RN) for an established Connecticut senior living and healthcare organization. The role focuses on evening admissions (3:00 PM – 11:00 PM) and coordinating clinical intake for new residents to ensure seamless transitions into the center.

Responsibilities include initial nursing assessments, medication reconciliation, and rapid electronic chart setup, enabling floor staff to provide direct resident care while upholding high clinical standards

Qualifications

• Active RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC or sub-acute rehab.
• Admissions experience or coordination experience desirable.
• EMR proficiency with PointClickCare for quick chart initiation.
• Strong organizational and interpersonal communication skills.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM – 11:00 PM).
• Perform initial nursing assessments, skin evaluations, and admission documentation.
• Reconcile hospital discharge summaries and orders for immediate medication continuity.
• Collaborate with discharge planners, care teams, and families for room setups and equipment.
• Initiate preliminary care plans and relay urgent resident needs to on-duty supervisors.
• Orient residents and families to facility policies and care routines.
