Job summary

KingRecruitingServices is seeking an Admissions Nurse (RN) for a Connecticut senior living and healthcare organization. The role oversees evening clinical intake, initial assessments, medication reconciliation, and rapid EMR chart setup to enable floor staff to focus on direct resident care.

The position coordinates with discharge planners, physicians, leadership, and families to ensure safe transitions with a high standard of clinical excellence. Evening shift, full-time, on-site.

Qualifications

• Active RN license in the State of Connecticut required.
• Minimum 1–2 years nursing experience in SNF/LTC or sub-acute rehab preferred.
• Experience handling admissions, discharge coordination, or desk nursing desirable.
• Hands-on EMR experience for rapid chart initiation (e.g., PointClickCare).
• Excellent organizational, prioritization, and interpersonal skills with families and hospital staff.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM).
• Conduct initial nursing assessments and admission documentation for new residents.
• Reconcile discharge summaries, orders, and pharmacy profiles to ensure medication continuity.
• Collaborate with discharge planners, care teams, and families to set up rooms and equipment.
• Develop preliminary individualized care plans and communicate needs to on-duty supervisors.
• Orient residents and families to policies and routines to ease transitions.
