The ideal candidate will hold an active CT RN license with 1–2 years in SNF/LTC settings, be proficient in PointClickCare, and possess excellent organizational and communication skills to partner with hospitals,
Qualifikationen
- Active, unrestricted RN license in Connecticut
- 1–2 years nursing experience in SNF/LTC/sub-acute setting preferred
- Experience with admissions, discharge coordination, or desk nursing desirable
- Hands-on EMR experience with PointClickCare for rapid chart initiation
- Strong organizational, prioritization, and interpersonal skills
Aufgaben
- Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM)
- Conduct initial nursing assessments and admission documentation
- Reconcile discharge summaries and orders to ensure medication continuity
- Coordinate with discharge planners, care teams, and families for room setup and equipment
- Initiate preliminary care plans and communicate needs to on-duty supervisors
- Orient new residents and families to policies and care routines
