Job summary

Ashton North LLC in Connecticut seeks an Admissions Nurse (RN) to oversee evening clinical intake for new residents. You will coordinate with discharge planners, physicians, facility leaders, and families to ensure safe transitions into the center.

Focus is on initial assessments, medication reconciliation, and initial electronic chart setup, enabling staff to concentrate on resident care from day one. CT RN license required; 1–2 years SNF/LTC experience preferred.

Qualifications

• Active, unrestricted Registered Nurse license in Connecticut.
• 1–2 years in SNF/LTC or sub-acute rehab settings preferred.
• Experience handling admissions, discharge coordination, or desk nursing desirable.
• Hands-on EMR experience (e.g., PointClickCare) for rapid chart setup.
• Strong organizational and interpersonal skills for families and hospital teams.

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3–11 PM).
• Perform initial nursing assessments and admission documentation on arrival.
• Reconcile hospital discharge summaries and orders for continuity.
• Coordinate with discharge planners, care teams, and families for room setup.
• Develop preliminary care plans and communicate needs to on-duty supervisors.
• Orient new residents and families to policies, routines, and care teams.
