Job summary

Northwestern Medicine, Palos Heights, IL, seeks a Staff Nurse to provide patient-centered care across the admission to discharge continuum. The RN collaborates with the care team to develop and implement individualized plans, educate patients, and document progress in a compliant, evidence-based manner.

The role requires a BSN or ADN with commitment to BSN, Illinois RN licensure, BLS, and area-specific certifications; experience in Emergency and Pediatric areas is noted as preference.

Qualifications

• BSN or ADN with commitment to obtain a BSN within three years of employment.
• Current State of Illinois Registered Professional Nurse license.
• BLS certification required by AHA.

Responsibilities

• Assess patient status at admission and identify care needs.
• Organize and document health data for care planning.
• Collaborate with patient, family and health care team on the plan of care.
• Initiate discharge planning in collaboration with patient and care team.
• Provide education based on evidence-based practice and track patient responses.
