Qualifications

Responsibilities

• Assess and record patient status and identify care needs on admission or at point of care.
• Organize assessment data to ensure accuracy, completeness and accessibility.
• Identify problems needing immediate attention and validate diagnoses with patient/family and health care team.
• Document diagnoses and plan of care to support outcomes and discharge planning.
• Initiate and update the Problem List; incorporate age-specific care into the plan.
• Provide patient/family education based on evidence-based practice and ongoing reassessment of care.
