Qualifications

Responsibilities

• Assess and record patient status on admission and at care points.
• Organize health data for accurate, accessible records.
• Identify problems needing immediate attention and communicate with team.
• Develop and update a plan of care aiming for realistic outcomes.
• Provide education to patient and family based on evidence-based practice.
• Document assessments, interventions, and patient responses.
• Participate in discharge planning and care coordination.
