Las Palmas Medical Center, part of HCA Healthcare, is seeking an ER Discharge Coordinator to manage Point of Care bedside scheduling as part of our centralized ER and Urgent Care Follow-up programs. The role focuses on creating a positive, patient-centered experience with timely follow-up and coordination.

You will speak with patients and team members, schedule follow-up care at bedside when appropriate, ensure quality standards, and contribute ideas to improve department policies.

Qualifications

• One year of related experience is preferred.

Job Summary and Qualifications

As an ER Discharge Coordinator, you willbe responsible forPoint of Care (PoC) Patient Bedside scheduling as part of the company's centralized ER and Urgent Care Follow-up programs. The truest intent of the ER discharge coordinator is a bedside-initiated patient experience, with theobjectivesof increased contact rate, scheduling immediacy, and improved overall patient experience. You willcreateandmaintaina positive customer experience in a patient-centered, quality-driven environment.

In this role you will:
