Job summary

UW Medical Center-Montlake seeks a Clinical Pharmacist for the Emergency Department, joining a premier inpatient team in Seattle. The role emphasizes drug therapy optimization, education, and transitions of care within a high-acuity environment.

The position requires an advanced degree or residency with strong hospital practice; PGY-2 critical care training is preferred. Work is per diem with rotating/variable shifts across Montlake and Harborview locations.

Qualifications

• Two years of experience in pharmacy practice.
• Residency or advanced degree may substitute month-for-month for experience.
• PGY-2 clinical or critical care training preferred.

Responsibilities

• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
• Utilize medical records to communicate with healthcare professionals and document pharmacotherapeutic recommendations.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and education.
• Update patient medication lists and reconcile medication orders at each visit.
• Provide primary preceptorship of pharmacy residents and student pharmacists.
• Perform duties of a Pharmacist 2.
