Job summary

UW Medical Center-Montlake in Seattle is seeking a part-time clinical pharmacist to join the inpatient multidisciplinary team. Responsibilities include drug therapy consults, protocol development, patient and staff education, transitions of care, and precepting students.

Candidates should have two years of clinical hospital pharmacy experience, or an ASHP-accredited residency, or equivalent. Washington state pharmacist license required.

Qualifications

• Two years of experience in pharmacy practice.
• ASHP-accredited residency training may substitute months for experience.

Responsibilities

• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
• Utilize the medical record and other documentation systems to communicate with healthcare professionals and document pharmacotherapeutic recommendations.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and education.
• Update patient medication lists and reconcile medication orders at each visit.
• Provide primary preceptorship of pharmacy residents and student pharmacists.
• Perform duties of a Pharmacist 2.
