The position emphasizes direct patient care, drug therapy consults, protocol development, and education for patients, families and staff. Prior residency or advanced degree is valued, with two years in hospital pharmacy practice as a baseline.

Qualifications

• Two years of experience in pharmacy practice.
• State of Washington Pharmacist License.
• PGY-2 critical care training preferred.

Responsibilities

• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
• Utilize the medical record and other documentation systems to communicate with healthcare professionals and to document pharmacotherapeutic recommendations.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visit.
• Provide primary preceptorship of pharmacy residents and student pharmacists.
• Perform duties of a Pharmacist 2.
