The role is fully remote, requiring collaboration with care managers, clinical leadership, and informatics teams to scale patient care while adhering to Epic best practices.

The candidate should bring 5+ years with a degree (or 7+ years without), Epic Healthy Planet certifications, and experience with Epic modules such as Ambulatory, Reporting Workbench, and Cogito.

Qualifications

• 5+ years of relevant experience with a degree (Required)
• 7+ years of relevant experience without a degree (Required)
• Epic Healthy Planet Clinical or Healthy Planet Analytics Certification Preferred
• Experience with EpicCare Ambulatory, Cogito, and other Epic modules is advantageous

Responsibilities

• Day-to-day support, build and optimization of Epic Healthy Planet care management and care coordination workflows and configuration
• Collaborate with care managers, clinical and operations leadership, and informatics teams
• Support quarterly update processes and uphold Epic best practices
