Job summary

Sentara Health is seeking an Epic Population Health IT Associate Analyst to support, test, and maintain Epic Population Health applications, including Healthy Planet and EpicCare Link, in support of our Population Health strategy.

This entry-level remote role partners with clinical and IT teams to analyze workflows, evaluate software releases, and resolve issues, with a focus on improving patient outcomes and clinician efficiency.

Qualifications

• Bachelor’s degree in a related field is required or equivalent experience.
• Epic HP (Healthy Planet) OR IP Clin Doc (Inpatient clinical Documentation) OR Epic Case Management Certification preferred.
• 3 years of relevant healthcare, clinical informatics, application support, or IT analyst experience with a degree.
• Experience in clinical case management, care coordination, or population health programs is required.
• Knowledge of inpatient nursing workflows and healthcare operations.
• Familiarity with Epic Healthy Planet, population health management tools, or healthcare technology applications preferred.
• Experience supporting software testing, workflow analysis, system upgrades, or application optimization activities preferred.
• Strong analytical, problem-solving, communication, and customer service skills.
• Ability to work effectively in a remote, collaborative team environment.

Responsibilities

• Provide day-to-day support, optimization, testing, and maintenance of Epic Population Health applications.
• Collaborate with clinical, operational, and technical teams to analyze workflows and support system enhancements.
• Evaluate software releases and resolve application-related issues.
• Support population health programs to improve patient outcomes, quality of care, and clinician efficiency.
