PriMed Management Consulting Services, Inc. is seeking a Senior Application Analyst to own Epic Healthy Planet configurations, care management workflows, and population health reporting.

You will partner with Care Management, Pharmacy, Quality, and IS teams to optimize patient registries and BPAs, ensuring regulatory and quality goals are met. The role requires 5–7 years in healthcare, Epic Healthy Planet proficiency, and strong communication.

Qualifications

• Experience 5–7 years in the Healthcare industry with Epic application knowledge.
• Current proficiency supporting Epic Healthy Planet or related Population Health applications.
• Experience with care management workflows, patient outreach, and quality improvement initiatives.
• Excellent written and verbal communication across clinical, operational, and technical teams.

Responsibilities

• Serve as primary application specialist for Epic Healthy Planet and related Population Health tools.
• Design, configure, support, and optimize population health workflows and care management.
• Gather business requirements and translate into technical solutions with cross‑functional teams.
• Provide training and guidance to colleagues and ensure system upgrades are tested and deployed with minimal disruption.
