CVS Health is seeking a Senior Epic Analyst to lead the design, build, and optimization of Epic Healthy Planet and Care Coordination solutions. You will mentor junior analysts and collaborate with population health leaders to translate program strategy into scalable Epic configurations.

You will design registries, care plans, and care teams, while ensuring effective risk stratification, care gap identification, and cross-continuum information exchange to support value-based care initiatives.

Qualifications

Bachelor’s degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience).
4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules.
Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred.
Demonstrated experience designing workflows for population health, care management, or value-based care programs.
Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Responsabilités

Design, build, and optimize Epic Healthy Planet solutions for population health and care coordination.
Lead testing and validation of Healthy Planet builds to ensure accuracy of registry logic and reporting outputs.
Design and maintain care coordination workflows supporting transitions of care and referral management.
Optimize interoperability workflows to support data exchange across care settings.
Partner with care management teams to streamline documentation, tasks, and communications within Epic.
Serve as Epic liaison for stakeholders translating program goals into technical requirements.
