Job summary

Fresenius Medical Care is seeking an entry-level MSW to provide psychosocial services to patients treated by the facility, including in-center and home dialysis where applicable. You will work with the care team to support adjustment, rehabilitation and quality of life.

Under supervision, you will assess needs, develop care plans, provide counseling, educate patients about transplant options, and connect families with community resources.

Qualifications

• Masters in Social Work required; licensure to practice in the state.
• State license required and applicable scope of practice.
• 0–2 years of related experience.

Responsibilities

• Assess psychosocial status and develop care plans with the healthcare team.
• Provide counseling and support to dialysis patients and families.
• Educate patients on transplant options and community resources.
• Document interactions and contribute to quality improvement activities.
• Participate in monthly quality review meetings with the interdisciplinary team.
• Coordinate referrals to home health, vocational rehabilitation, and social services.
• Assist with insurance and financial counseling related to dialysis care.
