Job summary

Fresenius Medical Care is seeking an entry level Medical Social Worker to provide psychosocial services to dialysis patients and families, including education and support to promote adjustment and quality of life.

In collaboration with the interdisciplinary team, you will conduct patient assessments, participate in care planning, and assist with referrals to community resources while adhering to policy and licensure requirements. Travel to home dialysis patients if applicable may be required.

Qualifications

• Master's in Social Work from an accredited program.
• State required license is required for this role.
• Meets applicable licensure requirements in the state of employment.

Responsibilities

• Assess patients' psychosocial status and participate in care planning with the interdisciplinary team.
• Provide counseling and education to patients and families to support rehabilitation and quality of life.
• Support transplant referral process and transplant waitlist management as part of education and counseling.
• Assist with referrals to community resources and vocational rehabilitation to enable employment or education.
• Document MSW interactions and interventions in accordance with policy.
