Job summary

Fresenius Medical Care seeks an entry level MSW to provide psychosocial services for dialysis patients in our facility. You will work with the care team to promote adjustment, rehabilitation, and quality of life, addressing barriers to treatment and coordinating with community resources.

Responsibilities include patient assessment, counseling, patient education on modalities and transplant options, and supporting continuity of care.

Qualifications

• Masters in Social Work and state license required.
• Meets applicable scope of practice and licensure requirements in the state of employment.
• 0–2 years of related experience in social work or patient support.

Responsibilities

• Assess patients' psychosocial status and develop care plans with the interdisciplinary team.
• Provide counseling and supportive services to patients and families.
• Coordinate referrals to community resources and transplant waitlist management.
• Educate patients about treatment options and advance directives as appropriate.
• Participate in quality improvement and regular interdisciplinary team meetings.
• Document interventions and maintain timely patient records.
