Job summary

Fresenius Medical Care is seeking an entry-level Social Worker (MSW) to provide psychosocial support for patients undergoing dialysis in the facility. You will work with the healthcare team to assess patient needs, educate families, and assist with referrals to community resources and transplant processes.

The role requires an MSW degree with state licensure and 0–2 years of related experience. Travel may be required for home dialysis patients where applicable.

Qualifications

• Knowledge of dialysis care is a plus.

Responsibilities

• Assess patients' psychosocial status and needs as part of comprehensive assessment.
• Participate in care planning with patient and healthcare team to meet rehabilitation and treatment goals.
• Provide counseling and education to patients and families about psychosocial issues.
• Assist with referrals to community resources and vocational rehabilitation as needed.
• Support patients adjusting to dialysis and transplant referral processes.
• Document interactions and maintain relevant policy adherence.
