Job summary

Fresenius Medical Care seeks an entry level MSW to provide psychosocial services to patients in our facility, including in-center and home dialysis patients when applicable. You will collaborate with the healthcare team to promote adjustment, rehabilitation and quality of life, educating staff on emotional and behavioral impacts of CKD.

You will support patient education, counseling, and referrals, contributing to quality indicators and improvement activities while documenting all interactions

Qualifications

• Masters in Social Work is required and must be from an accredited program.
• Must have a state-required social work license to practice.

Responsibilities

• Assess patients' psychosocial status and plan care with the interdisciplinary team.
• Provide counseling and educational support to patients and families.
• Coordinate with community resources and transplant referrals as needed.
• Document interactions and ensure adherence to policies and quality goals.
• Support staff with education about psychosocial issues and patient rights.
