Fresenius Medical Care is seeking an entry-level Medical Social Worker to deliver psychosocial services to dialysis patients in center and home settings. You will collaborate with the care team to support adjustment, rehabilitation, and quality of life for patients and families.

The role emphasizes assessment, counseling, education about treatment options, transplant referrals, and coordination with community resources. 36 hours/week; requires an MSW and state licensure.

• Performs psychosocial assessments and counseling within state licensure.
• Works with interdisciplinary team to plan patient care and rehabilitation.
• Provides patient education on psychosocial issues related to ESRD and dialysis.
