Job summary

Fresenius Medical Care is seeking an entry level Master of Social Work to provide psychosocial services for in-center and home dialysis patients. You will work with an interdisciplinary team to promote adjustment, education, and quality of life for patients and their families.

The role requires a Master’s in Social Work, state licensure, and 0–2 years of related experience. Travel and home dialysis support may be involved where applicable.

Qualifications

• Masters in Social Work required.
• Must have state license.
• Meets the applicable scope of practice board and licensure requirements.
• 0–2 years related experience.

Responsibilities

• Patient Assessment / Care Planning / Counseling in collaboration with the interdisciplinary team.
• Provide monitoring and interventions to support adjustment to dialysis.
• Educate patients and families about treatment options and support services.
• Assist with referral to community resources to improve quality of life.
• Document MSW interactions and interventions according to policy.
