Qualifications

• MSW degree required and state licensure for Social Work.

Responsibilities

• Assess psychosocial status and plan care with rehabilitation goals.
• Provide counseling to support adjustment to dialysis and improve quality of life.
• Educate patients on treatment modalities and transplant referrals; assist with advance directives.
• Coordinate with Insurance Coordinators on coverage and financial assistance.
