Habitat Health in California is seeking an Enrollment RN Per‑Diem to lead the clinical enrollment process for new participants. You will conduct eligibility and home safety assessments in participants' homes, educate families, and support informed enrollment decisions with the IDT.

Working with Outreach and Enrollment Specialists, you will document findings in the EHR, ensure compliance with DHCS, and help create a welcoming, participant‑centered enrollment experience while supporting growth.

Qualifikationen

• Active RN license in the applicable state.
• Experience conducting clinical and home safety assessments.
• Ability to learn enrollment management and clinical documentation systems.
• Strong communication with participants, caregivers, and cross-functional teams.

Aufgaben

• Conduct nursing, home safety, and eligibility assessments in participants' homes or care settings to inform enrollment decisions.
• Complete NFLOC assessments and documentation per DHCS requirements.
• Present clinical findings at enrollment eligibility meetings with the IDT.
• Maintain accurate clinical documentation in the EHR and related systems.
• Build trusted relationships by educating and addressing questions from participants and caregivers.
• Travel throughout the service area as needed and stay compliant with regulatory requirements.
