Qualifications

• 2 years of healthcare experience required; 8 months of previous clinical experience preferred.
• 1 year of telephonic or call center customer service experience; call center experience highly preferred.
• Experience in medical practice or healthcare setting with scheduling and documentation in EHR systems.

Responsabilités

• Conduct targeted telephonic outreach to patients and families educating them on palliative care and VyncaCare services.
• Complete structured eligibility screenings to identify patients with chronic illness and functional decline.
• Schedule eligible patients for an initial nurse practitioner visit and coordinate handoff to care.
• Coordinate enrollment efforts with market and business development teams to drive census growth.
