Vynca is seeking a compassionate Enrollment MA to identify and enroll patients into our palliative care program. You will conduct outreach calls to patients, families, and healthcare organizations, perform structured eligibility screenings, and schedule initial nurse practitioner visits.

Build relationships with health plans, offices, discharge planners, and community partners to grow census and program reach.

Competenze

• CMA or CCMA certificate issued by a recognized credentialing body.
• 2 years of healthcare experience required; 8 months of clinical experience preferred.
• 1 year of telephonic or call center customer service experience; call center experience highly preferred.
• Experience in a medical practice or healthcare setting with familiarity with EHR documentation and scheduling.
• Experience and training in motivational interviewing and patient education.
• Strong organizational skills; ability to work independently with high-volume outreach.
• Proficient with Microsoft and Google applications; reliable internet connection.
• Strong written and verbal communication skills.
• Exceptional interpersonal skills including conflict resolution and de-escalation.
• Willing to work Monday–Friday, 8:30am–5pm in various US time zones; overtime possible.

Mansioni

• Conduct targeted telephonic outreach to patients from provided lists with high-quality, patient-centered communication.
• Confirm service area eligibility and complete structured screenings capturing demographics, chronic disease history, and functional status.
• Educate patients and families on palliative care and VyncaCare services in accessible language.
• Schedule eligible patients for an initial nurse practitioner visit and coordinate handoff to care.
• Act as liaison between patients and care team; direct calls, texts, and emails to appropriate staff.
• Maintain patient confidentiality and accurate documentation per HIPAA and policies.
• Coordinate enrollment efforts across regions; track outreach activity to drive census growth.
• Identify outreach trends and barriers; share feedback with market and business development teams.
