Job summary

Vynca Inc. is seeking an Enrollment Medical Assistant to lead patient identification and enrollment into the palliative care program.

You will conduct targeted outreach by phone to patients, families, and healthcare organizations, educate on palliative care, complete eligibility screenings, and schedule initial nurse practitioner visits. Your role requires motivational interviewing and clear, compassionate communication to support patients and caregivers.

Qualifications

• Current CMA or CCMA certificate issued by a recognized credentialing body.
• Minimum 2 years of healthcare experience; 8 months preferred.
• At least 1 year of telephonic or call center customer service.
• Experience in a medical practice with EHR documentation.
• Training in motivational interviewing and patient education.

Responsibilities

• Conduct targeted telephonic outreach educating patients and families about palliative care.
• Screen patients for eligibility using structured forms and coordinate nurse practitioner visits.
• Educate patients on services and what to expect from the program.
• Schedule initial visits and coordinate handoffs to care teams.
• Serve as liaison between patients and care team, handling calls and messages.
• Maintain HIPAA-compliant documentation and confidentiality.
• Collaborate with market teams to identify outreach trends and barriers.
