Job summary

Vynca Inc. is seeking an Enrollment MA to identify and enroll patients into our palliative care program. You will educate patients and families, conduct structured screenings, and schedule initial visits with a nurse practitioner.

You will build relationships with health plans and providers, drive census growth, and collaborate with market and business development teams to improve program reach. Fluency in English/Spanish is preferred.

Qualifications

• Minimum CMA or CCMA certification.
• Approximately 2 years healthcare experience required.
• At least 1 year of telephonic or call center customer service experience.
• Experience in medical practice or healthcare setting; familiarity with EHR platforms.
• Experience and training in motivational interviewing and patient education.
• Strong organizational skills; ability to work independently with high-volume outreach.
• Proficient with Microsoft and Google applications; standard work hours 8:30am-5pm ET with possible overtime.
• Bilingual English/Spanish.
• Medical terminology certification.
• Familiarity with palliative care, home-based care, or chronic disease management.

Responsibilities

• Conduct targeted telephonic outreach to patients and families.
• Confirm service area eligibility and complete structured screenings.
• Educate patients on palliative care and VyncaCare services.
• Schedule eligible patients for an initial nurse practitioner visit and coordinate handoff.
• Act as liaison between patients and care team; route inquiries promptly.
• Coordinate enrollment efforts across regions and track outreach activity.
• Provide qualitative and quantitative feedback to inform strategy.
