Job summary

Vynca Inc. is seeking an Enrollment MA to serve as the primary contact for enrolling patients into our palliative care program.

You will conduct targeted outreach to patients, families, and healthcare organizations, educate them on VyncaCare's services, and screen for eligibility to identify candidates for an initial nurse practitioner visit. You’ll collaborate with health plans, physician offices, discharge planners, and community partners to drive census growth, document interactions in EHRs,

Qualifications

• Current CMA or CCMA certification.
• 2 years of healthcare experience required; 8 months of previous clinical exposure preferred.
• 1 year of telephonic or call center customer service experience; call center experience highly preferred.
• Experience working in a medical practice or healthcare setting; familiarity with EHR documenting patient interactions and scheduling appointments.
• Experience and training in motivational interviewing and patient education; comfortable discussing serious illness and chronic disease with patients and families.
• Strong organizational skills and ability to work independently with high-volume outreach.
• Proficiency with Microsoft and Google applications; reliable internet connection required.
• Strong written and verbal communication skills; exceptional interpersonal skills including conflict resolution.
• Availability Monday through Friday, 8:30am–5pm in ET/CT/PT time zones; overtime possible.

Responsibilities

• Conduct targeted telephonic outreach to patients on provided lists, delivering high-quality patient-centered service.
• Confirm service area eligibility and complete structured screening forms capturing demographics, chronic disease history, and functional decline indicators.
• Educate patients and families on palliative care and VyncaCare's services, translating complex concepts into accessible language.
• Schedule eligible patients for an initial visit with a nurse practitioner and coordinate logistics for a smooth handoff to care.
• Act as a liaison between patients and the broader care team, routing calls, texts, and emails appropriately and responding to questions.
• Maintain confidentiality and accuracy in patient documentation in accordance with HIPAA and organizational standards.
• Coordinate enrollment efforts across targeted regions, tracking outreach activity to encourage referrals and census growth.
• Identify outreach trends, surface barriers to enrollment, and share feedback with the market and business development team to improve program reach.
• Maintain a collaborative relationship with the market team and practice leadership to support program expansion and patient access.
