Job summary

United Health Centers of the San Joaquin Valley in Fresno is seeking a Patient Care Coordinator to help patients enroll in Medi-Cal and other health plans, and to arrange follow-up care after discharge for continuity of care.

You will educate patients about coverage, assist with applications, verify information, and navigate programs while delivering excellent customer service and maintaining confidentiality in a busy hospital setting.

Qualifications

• Bilingual required.
• Clear verbal and written communication skills.
• Prior experience with an electronic medical record system.
• Strong knowledge of medical billing and revenue cycle.
• Understanding of California and Federal payer regulations.
• Ability to handle multiple tasks simultaneously.

Responsibilities

• Schedules follow-up care and updates patient information.
• Screen Medi-Cal and other programs eligibility.
• Assist applicants with completing applications.
• Maintain confidentiality of all applications.
• Explain required enrollment documents.
• Refer applicants to DSS for Medi-Cal when needed.
• Assist with estimating monthly premiums.
• Manage revenue cycle data and patient estimation.
• Answer calls promptly and route messages.
• Maintain patient schedules for enrollment and referrals.
• Provide outstanding customer service.
• Report concerns to Enrollment Manager and Director of Billing.
• Review enrollment docs and follow-up.
• Build community relationships for future enrollments.
• Promote teamwork and positive communications.
