Responsibilities

• Conduct patient outreach and education via telephone, text, and patient portal regarding health assessment and care continuity.
• Coordinate PCP assignment using the Four-Cut Method.
• Connect patients to health services according to standards.
• Screen patients for Social Determinants of Health (SDOH).
• Provide community resources or plan resources as appropriate.
• Participate in quality improvement projects and report outcomes.
