MasterCare, Inc. is expanding its team with a Care Navigator to provide non-clinical care management under the MasterCare Plan. You will engage patients in person and remotely, coordinating with providers and social resources to address holistic needs.

The role emphasizes building relationships, conducting comprehensive assessments, and developing tailored care plans for Enhanced Care Management and Housing Navigation within California. Travel across assigned territory is required.

Qualifications

• Bachelor’s degree or equivalent in healthcare-related fields.
• 3+ years in healthcare marketing or social services.
• Experience with elderly care services and housing options.

Responsibilities

• Be primary contact with local medical and nonmedical providers.
• Develop and foster professional relationships; conduct outreach and education.
• Develop referrals and placement networks to reach company goals.
• Assist in provider relations and local resource development.
• Conduct comprehensive assessments for ECM and CS patients.
• Develop and execute the Master Care Plan for patients.
• Respect patient goals and coordinate care to improve health.
