Horizon Center San Diego is seeking an Enhanced Care Manager to engage and support ECM program members, coordinating care with providers and discharge planning as needed.

The role focuses on monitoring treatment adherence, health promotion, and developing individualized plans while maintaining thorough records and professional communication with stakeholders in a compliant environment.

Formación

• Proven written, oral, and interpersonal communication in English.
• Knowledge and experience in Behaviour Management; interact effectively with individuals.
• Ability to work independently within agency policies and procedures.

Responsabilidades

• Engage and support ECM program members.
• Monitor adherence to Care Plans aligned with provider recommendations.
• Conduct health promotion and self-management training.
• Maintain regular communication with members, including home visits if necessary.
• Build and nurture relationships with internal and external stakeholders.
• Assist with discharge planning and transition of care.
• Coordinate care with case management staff and community providers during emergencies.
• Attend medical appointments to ensure continuity of care.
• Document interactions and maintain referral records to support services.
• Develop psychosocial assessments and collaborative treatment plans.
• Perform ECM-related duties as assigned.
• Ensure HIPAA compliance and confidentiality of member information.
• Adhere to local, state, and federal regulations.
• Contribute to quality assurance/improvement activities.
• Utilize Electronic Health Record systems efficiently.

Qualifications:

• California Advancing and Innovating Medi-Cal (CalAIM) initiative experience.
• Certificate or equivalent in a related human/social service field with emphasis on community living services.
• One year of recent related experience in community living. Exhibits a healthy attitude towards normalization principles and is aware of their position as a positive role model.
• Demonstrated ability in written, oral, and interpersonal communication skills in English.
• Have knowledge of and experience in Behaviour Management. Demonstrated ability to interact effectively with the individuals.
• Demonstrated ability to work independently within established agency policies and procedures.
