Job summary

Horizon Center San Diego is seeking an Enhanced Care Management (ECM) professional to support patients in a recuperative care setting. You will engage members, monitor adherence to Care Plans, and coordinate with providers to ensure continuity of care.

This role emphasizes compassionate service, HIPAA compliance, and strong communication. You will work with a dedicated team to discharge planning and transitions, while documenting interactions and referrals within the electronic health record.

Qualifications

• CalAIM experience required.
• Certificate or equivalent in related field with emphasis on community living services.
• At least one year of recent related experience in community living.
• Strong written and spoken English communication.
• Knowledge of Behaviour Management and positive role modelling.
• Ability to work independently within agency policies.

Responsibilities

• Engage and support members in the ECM Program.
• Monitor treatment adherence through Care Plans aligned with provider recommendations.
• Conduct health promotion and self-management training.
• Maintain regular communication with members, including home visits if necessary.
• Build and nurture relationships with internal and external stakeholders.
• Assist with discharge planning and transition of care.
• Coordinate care with case management staff and community providers during emergencies.
• Attend medical appointments to ensure continuity of care.
• Document interactions and maintain records of referrals to various support services.
• Conduct psychosocial assessments and develop treatment plans collaboratively.
• Perform other ECM-related duties as assigned.
• Ensure HIPAA compliance and treat member information confidentially.
• Adhere to local, state, and federal regulations.
• Contribute to quality assurance/improvement activities.
• Utilize Electronic Health Record systems efficiently.

Qualifications:

• California Advancing and Innovating Medi-Cal (CalAIM) initiative experience.
• Certificate or equivalent in a related human/social service field with emphasis on community living services.
• One year of recent related experience in community living. Exhibits a healthy attitude towards normalization principles and is aware of their position as a positive role model.
• Demonstrated ability in written, oral, and interpersonal communication skills in English.
• Have knowledge of and experience in Behaviour Management. Demonstrated ability to interact effectively with the individuals.
• Demonstrated ability to work independently within established agency policies and procedures.
