Job summary

Affinity Health Management is seeking a Clinical Care Coordinator to provide administrative support to the Clinical Care team by managing the electronic medical records system and supporting back-office functions including billing, payroll, and HR.

The role includes training new coordinators and ensuring data integrity across information systems, while maintaining PHI confidentiality and coordinating with the Clinical Manager and Regional team.

Qualifications

• Two years health care experience, preferably in hospice.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Proficient in MS Office and Google Applications.
• HCHB/Pointcare knowledge is preferred.

Responsibilities

• Accurately enters a variety of data into various information systems.
• Executes workflow functions in electronic medical record system.
• Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director.
• Manages incoming, outgoing and interoffice mail.
• Provides customer support to patients and colleagues via phone, email and fax.
• Alerts appropriate management team members regarding potential compliance issues.
• Maintains confidentiality of PHI and organization information at all times.
• Order DME and alerts DME supplier when equipment is no longer in use/needs to be exchanged for other, etc.
• Participates and scribes IDG meetings.
• Performs other specific projects relating to office procedures.
• Provides training and ongoing support to new Clinical Care Coordinators within the region.
