Job summary

You will complete progress notes, participate in meetings, maintain records, and assist new staff with orientation and training.

Qualifications

• High School Diploma required.
• Valid US driver’s license.
• Experience working with adults with a mental health diagnosis and/or developmental disabilities.

Responsibilities

• Assess individuals to understand hopes, dreams, and goals and provide supports to help achieve them.
• Protect and promote the rights of individuals receiving services and support advocacy efforts.
• Serve as a resource on community agencies, services, and supports meeting identified needs.
• Supervise, educate, and monitor individuals in work, home, school, or community settings as needed.
• Provide one-on-one support to meet emotional, physical, and medical needs; document progress and outcomes.
