Job summary

Responsibilities

• Assess individuals’ hopes, dreams, and desires and provide supports that facilitate achievement.
• Protect and promote the rights of individuals receiving services and assist with advocacy.
• Serve as a resource about community services and supports to meet identified needs and goals.
• Supervise, educate, and monitor individuals in work, home, school, or community settings.
• Provide one-on-one support to meet emotional, physical, and medical needs.
