Job summary

Klamath County Developmental Disabilities Services is seeking a Services Coordinator to provide targeted case management for individuals with intellectual/developmental disabilities (I/DD). You will identify needs and develop plans to access services and resources.

The role requires developing and revising ISPs, coordinating with providers, and ensuring Medicaid standards are met, while fostering self-determination and person-centered care in a county setting.

Qualifications

• Knowledge of the public service system for developmental disabilities services in Oregon.
• Bachelor's degree in behavioral science, social science, or a closely related field; or Bachelor’s degree in any field with 1 year of human services related experience; or Associate’s degree with 2 years of related experience.
• Ability to participate in a core competency training sequence approved by ODDS and KCDDS.

Responsibilities

• Assess and reassess an eligible individual's service needs to determine ISP requirements.
• Develop and periodically revise an ISP/Annual Plan based on assessments.
• Access services via referrals and monitor Medicaid Plan of Care.
• Deliver person-centered services and support self-determination.
• Maintain confidentiality and professional boundaries; complete 20 hours of I/DD training annually.
