Job summary

WellLife Network is seeking a Direct Support Professional in New York on a per diem basis. You will provide hands-on care, advocacy and support to adults, helping with daily living activities and community engagement.

The role emphasizes collaboration with healthcare teams, documentation in electronic systems, and promoting dignity and independent living for individuals with developmental disabilities or behavioral health needs.

Qualifications

• High School Diploma or equivalency required.
• Fluent in English for client communication.

Responsibilities

• Assist with activities of daily living including bathing, dressing, grooming, and toileting.
• Administer medications as directed and support medical appointments and treatment plans.
• Assist with walking, transfers, and use of mobility aids; ensure safe navigation of environments.
• Implement strategies to support behaviors respectfully and provide encouragement for independence.
• Plan and participate in recreational, educational, and social activities; encourage community involvement.
• Assist with communication tools or techniques to help individuals express themselves; advocate for their needs and rights.
• Collaborate with healthcare providers, therapists, and team members; maintain accurate daily notes and reports.
