Job summary

Breakthroughs LLC is seeking a dependable Supported Living Coach to provide coaching and support to adults with developmental disabilities living independently. This full-time role manages a caseload of about 8 clients, offering up to 5 hours per week per client at $25/hour, with onboarding training through Train.org.

Ideal candidates will have strong organizational skills and be adept with electronic documentation, APD iConnect, and HIPAA compliance.

Qualifications

• Current SLC certificate OR ability and willingness to obtain an SLC certificate.
• Strong organizational and time-management skills.
• Comfortable completing online training and using electronic systems for documentation.

Responsibilities

• Provide individualized Supported Living Coaching based on each consumer's approved support plan and goals.
• Teach and support individuals in developing skills that promote greater independence.
• Assist with daily living skills, community participation, budgeting, safety, decision-making, self-advocacy, and accessing community resources.
• Maintain a consistent schedule across an assigned caseload.
• Complete accurate, timely, and objective documentation for every service provided, including APD iConnect.
• Communicate professionally with consumers, families/guardians, Support Coordinators, and other team members.
• Maintain confidentiality and follow HIPAA, APD, Medicaid Waiver, and agency requirements.
