Job summary

Breakthroughs LLC is seeking a dependable Supported Living Coach to provide coaching and support to adults with developmental disabilities living independently. This full-time role manages a caseload of about 8 clients, offering up to 5 hours per week per client at $25/hour, with onboarding training through Train.org.

Ideal candidates will have strong organizational skills and be adept with electronic documentation, APD iConnect, and HIPAA compliance.

Responsibilities

• Maintain a consistent schedule across an assigned caseload.

Job description

Breakthroughs LLC is seeking a dependable, compassionate, and organized Supported Living Coach (SLC) to provide coaching and support to adults with developmental disabilities who live independently in their communities.

Position: Full-Time Supported Living Coach

This is a full-time, caseload-based position. The SLC will manage a caseload of approximately 8 consumers, with each consumer authorized for up to 5 hours of Supported Living Coaching per week at $25/HR, based on their individual service authorization and needs.

The position requires strong organization, time management, and documentation skills. You will work with multiple consumers throughout the week and complete required documentation for each visit.

SLC Certification Required

You must either already have an SLC certificate or be eligible and willing to obtain one.

All required SLC courses are available through Train.org, and new SLC staff will be expected to complete the applicable courses and training requirements as part of the onboarding process.

Applicants should be comfortable completing online training and using electronic systems for required documentation.

Additional Hours Available

Consumer needs and service authorizations can change, so the number of available SLC hours may vary.

If a full caseload of approximately 8 SLC consumers is not available for any reason, additional Personal Supports hours may be available in the same area at $18/hour. This provides an opportunity for qualified staff to supplement their SLC caseload and work toward full-time hours when additional Personal Supports hours are available.

• Communicate professionally with consumers, families/guardians when applicable, Support Coordinators, and other members of the individual's support team.
• Maintain confidentiality and follow HIPAA, APD, Medicaid Waiver, and agency requirements.
• Report incidents and concerns according to required procedures.
• Complete required APD, SLC, and agency training.
• Maintain professional boundaries and provide services in accordance with each consumer's approved plan.

Documentation Is a Major Part of This Position

Supported Living Coaching is not simply a hands-on support role. Documentation is a significant and required part of the job.

SLC staff must be able to accurately document each visit, including the services provided, supports delivered, progress toward individual goals, and any necessary follow-up.

Because you may be working with approximately 8 consumers, strong organization and the ability to keep each consumer's documentation accurate and up to date are essential.

Training & Onboarding

Breakthroughs LLC will provide guidance throughout the onboarding process. Staff who need to complete SLC training will be directed to the applicable Train.org courses and must successfully complete all required training before providing services that require those qualifications.
