Job summary

Caliber Therapies is seeking a Life Skills & Community Support Professional to work one-on-one with clients in homes and communities, helping them gain independence and meaningful participation in everyday life.

In this non-clinical role, you will coach, mentor, and provide hands-on support to a diverse client base, promoting confidence, daily living skills, and community involvement while maintaining professional boundaries and documentation.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years of age.
• Valid driver's license, reliable transportation.
• Ability to pass all required criminal background checks.
• Ability to work independently in home and community settings.
• Strong verbal and written communication skills.
• Basic computer proficiency and ability to complete electronic documentation.
• Ability to maintain professional boundaries and client confidentiality.

Responsibilities

• Build positive, professional relationships with clients and their support systems.
• Assist individuals in developing and practicing independent living skills.
• Support participation in community outings and recreational activities.
• Teach and reinforce communication, problem-solving, decision-making, and social skills.
• Assist clients in identifying and achieving person-centered goals.
• Encourage healthy lifestyle habits, physical activity, and emotional wellness.
• Support participation in volunteer opportunities, educational activities, employment readiness, and community involvement when appropriate.
• Promote self-advocacy, confidence, and independence while providing the least restrictive support necessary.
• Maintain accurate and timely documentation according to company and funding source requirements.
• Communicate professionally with caregivers, case managers, and the Caliber Therapies team.
• Follow all company policies regarding safety, confidentiality, professionalism, and mandatory reporting.
