Job summary

Helpline Youth Counseling is seeking a Rapid Re-Housing Case Manager to provide housing-focused case management to high-acuity clients within the Long Beach Rapid Re-Housing program. Role emphasizes trauma-informed care and coordinating services to promote housing stability.

The position requires collaboration with housing partners, ability to travel to client locations, and delivering client-centered support for up to 24 months as funding allows.

Qualifications

• Trauma-informed care knowledge and skills.
• Motivational interviewing techniques.
• Understanding of harm reduction and housing first principles.
• Strong communication skills, both written and verbal.
• Ability to manage time and multiple tasks with deadlines.

Responsibilities

• Receive referrals and coordinate intakes promptly.
• Process and update rental packets for each client.
• Provide trauma-informed case management to a caseload.
• Maintain a caseload averaging about 20 clients.
• Develop individualized Housing Retention Plans.
• Develop budgets at intake and every 90 days.
• Assist clients in increasing income to sustain housing.
• Collaborate with property managers and other services.
• Provide warm handoffs to external services and ensure connections to needed supports.
• Conduct at least one home visit per month and follow up on goals.
• Maintain accurate service records and HIPAA-compliant information.
• Participate in regular management check-ins and meetings.
• Perform other duties as assigned by leadership.
