Job summary

HYC, INC. in Huntington Park, CA seeks a Rapid Re-Housing Case Manager to provide housing-focused case management for high-acuity clients under the Long Beach RRH program.

This role uses trauma-informed care and motivational interviewing to help clients achieve housing stability within a 24-month window. You will manage a caseload of about 20 clients, develop individualized Housing Retention Plans, assist with budgeting and income enhancement, and coordinate with property managers and community

Qualifications

• High School Diploma; one year experience with people experiencing homelessness or equivalent.
• Knowledge of case management techniques including trauma-informed care, motivational interviewing, harm reduction.
• Be willing to work flexible hours; travel to clients; reliable transportation.
• HMIS experience is highly desirable.

Responsibilities

• Receive referrals and coordinate intakes promptly.
• Process and update rental packets for each client.
• Provide trauma-informed case management services to clients on their caseload.
• Maintain a caseload averaging 20 clients.
• Develop comprehensive Housing Retention Plans addressing goals.
• Develop budgets every 90 days with clients.
• Assist clients to increase income to sustain housing.
• Collaborate with property managers and other organizations.
• Conduct at least one home visit per month and follow up.
• Maintain confidential records and HIPAA compliance.
• Participate in meetings and other duties as assigned.
