Job summary

Bridge Over Troubled Waters in Boston, MA is seeking a Rapid Rehousing Case Manager to support homeless youth aged 18–24 in the Rapid Rehousing Program. You will assess housing barriers and develop individualized plans to help residents obtain permanent housing and access community services.

Responsibilities include housing assessments, managing a caseload of about 30 participants, documenting progress, coordinating with service providers, and ensuring culturally respectful supports.

Qualifications

• 2-4 years of experience working with vulnerable youth ages 18–24.
• Experience with housing programs or housing first philosophy.
• Knowledge of data collection and management systems.
• Knowledge of social services and basic benefit systems.
• Valid driver’s license and clean driving record.

Responsibilities

• Conduct housing needs assessments to connect residents to services.
• Assess housing barriers and develop individualized service plans.
• Provide case management to a caseload of about 30 participants.
• Maintain client files and monthly progress reports.
• Assist youth with locating community resources and housing options.
• Collect and report program data for funders and internal systems.
• Ensure services are culturally sensitive and inclusive.
