Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities under the Clinical Manager. You will develop plans of treatment, address disorders of speech, voice, language and swallowing, and document progress with professional care.

The ideal candidate has at least six months of experience, an unrestricted license, CPR certification, and reliable transportation.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver‘s license, auto insurance, and reliable transportation are required.
• Home Health experience (preferred)

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening
• Observe, record and report changes in the patient‘s condition and response to treatment
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation and reports as required by Company policy
• Participate in care coordination activities and discharge planning
• Maintain confidentiality and proper information sharing
