Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver home-based care, evaluate patients and contribute to care plans. You will work to improve communication, swallowing and overall functional abilities, coordinating with the clinical team.

The role requires 6 months of experience, an unrestricted license, CPR certification, and a valid driver's license with reliable transportation. Benefits include comprehensive coverage and disability protection.

Qualifications

• 6 months minimum experience as a speech therapist / speech language pathologist.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in the patient’s condition and response to treatment.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility; prepare and submit written reports.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality of patient information in accordance with policy.
